to the Humane Society's adoption
program. The information on the following pages is
requested so that we can assist you in the selection of a
new pet. These forms and consultation with a Humane
Society representative are designed to help us determine if
the adoption is in the best interest of the
animal; and to assist you in finding an animal most
compatible with your lifestyle.
In order to be
considered as an adopter you must:
Be 21 years of age or older (unless employed full-time and
responsible),
Have photo
identification showing your present address,
Have a fenced yard,
(preferably 6 ft). Some exceptions can be made for dogs under 10 lbs. who do not move as quickly
as larger dogs.
Have the knowledge and
consent of your landlord if you rent,
Be able and willing to
spend time and money necessary to provide training, medical treatment, and proper care for a
pet.
Please note that all
animals will be spayed/neutered before they leave our
sanctuary. A home visit is required prior to adoption.

retains the right to refuse to adopt an animal to
any person where they feel the adoption would not be
in the best interest of the animal.
